Decontamination of Instruments

In order to secure the good health and safety of our employees, we are asking you to take note of the fact that in accordance with our condition of service, you are obligated to ensure that the instruments you send to Miltenyi Biotec for service are decontaminated and do not represent a hazard to health. You signify your acceptance of these legally binding conditions when you place an order with us.
We are therefore asking you to lay a copy of the provided Decontamination Declaration to each instrument being returned. The Decontamination Declaration serves as information for our service technicians as to the condition of the instrument, thus enabling them to take any precaution necessary before opening the transport box. Please include the fully completed and signed Decontamination Declaration among the shipping documents attached to the instrument.
For safety reasons, Miltenyi Biotec reserves the right to either decontaminate at cost to the customer, or to reject acceptance of, all and any instrument sent for service that does not have attached a copy of the fully completed and signed Decontamination Declaration, and/or those instruments reasonably presumed to be contaminated and may therefore represent a health hazard.
